As your local federal member of parliament I can help you with enquiries that concern Commonwealth Government Departments. Please contact my office on 08 9527 9377 or email me at gary.gray.mp@aph.gov.au
Congratulatory Messages
If you live in Rockingham, Kwinana or Mandurah, then my office will be able to help you apply for a congratulatory message to be sent to yourself or someone you know who is about to celebrate a significant birthday or wedding anniversary. If you don't live in Rockingham, Kwinana or Mandurah, then you need to contact your local member to arrange a congratulatory message.
The following notifications are available:
50th wedding anniversary
Prime Minister
Governor General
50th plus wedding anniversary
Prime Minister
Governor General
The Queen (60th and every fifth year thereafter)
90th birthday
Prime Minister

Prime Minister
Governor General (100th birthday)
The Queen (100th and every fifth year thereafter)
Forms are available from my office. Please make sure you bring along a copy of the birth certificate or a copy of the wedding certificate, whichever is applicable.
If you can't get a copy of these certificates, you will need to obtain a statutory declaration stating the full name of the recipients, their current address and the date of the birthday or wedding anniversary.
You will need to allow at least four weeks before the date of the birthday or wedding anniversary so as to ensure that the message can arrive on time.
